Fellowship Connect Pre-Release and Release Application

This online application is provided for those who did not have an opportunity to fill out a questionaire before release, or by chaplains, counselors and case workers who are helping the applicant to submit this form prior to their release.

Forms are also provided at correctional facilities in the chapels for pre-release application. It is important that they submit the application themselves through the proper channels. The online submission will only be accepted from the applicant themselves or a chaplain, counselor, or case worker they have requested to submit it. Submission information for the online forms will be verified by Fellowship Connect.

Please notice that not all fields are required, but the more information you provide Fellowship Connect the more closely we will be able to match you with the type of fellowship you are seeking. When the application is completed, click on the submit button. You will be contacted by ministry teams from the types of fellowship interests you have preferenced.

You may submit an online form for removal from the process at any time.
